All Student-Athletes MUST have an updated physical exam for the current school year to participate. The official OSSAA Physical and Medical History Form can be found HERE.


Parents of student-athletes must also complete the RankOne Sport online paperwork in order for their athlete to participate.

 
Parents and athletes should also complete the Travel Permission Form, and return it to their head coach.
